SwiftDoc has closed. We understand this is disruptive, and we want to make the transition as smooth as possible.

Link Healthcare has acquired the practice's Socrates patient management licence, and your medical records have already been transferred to our system to allow continuity of care at our Ballincollig clinic.

Your records have been transferred as part of this transition. In line with GDPR, you have the right to request that your records be sent to a GP of your choosing instead, and you may request a copy of your data at any time. Records are stored securely and only accessed by clinical staff involved in your care.

If you'd like your records sent there, please ask your new GP to send us a signed consent form authorising the transfer, along with your full name, date of birth, and previous SwiftDoc registration details.
